Methodist churches throughout the conference have the opportunity to help create healthier populations, thanks to a collaboration between the Texas Annual Conference and Houston Methodist Hospital.
Statistically, a healthcare provider often can predict which patients are more likely to have complications that require them to come back in to the hospital. This likelihood of readmission often is due to problems such as a lack of healthy food, not being able to afford medications, or not going to follow-up appointments due to a lack of transportation.
